New Student Registration

Welcome to Mountain View Elementary!

In order to complete the registration process please complete, print, and submit the following information to the Mountain View Front Office. Please make sure all necessary documentation is included.

Your child will not be assigned a Mountain View teacher or class until the REGISTRATION PROCESS IS COMPLETED.

The following documents MUST be provided at the time of enrollment:

  1. Student Information Form – This is a fillable form, which means you can type in your information and then print it out. Parents need the last 4 digits of their social security number. We need a current email address for teachers to contact parents.
  2. Immunization Record Please provide the office with an original immunization record. If you student has not completed the required immunizations, they must be completed before starting school.
  3. Birth Certificate – Can NOT be wallet size birth card. You can obtain a copy from your current school or the Health Department.
  4. Proof of Residency  – MUST provide one document from Column A and one document from Column B or two documents from Column B and fill out the right hand side of the paper.
  5. Homeless Assistance Questionnaire (McKinney–Vento Eligibility) – Answer the two questions on the form.
  6. Parent/Guardian ID
  7. Guardianship Form – Fill out and sign.
  8. Emergency Release Form – Fill out and sign the form in the office.
  9. I.E.P – (Resource/Speech/Preschool/Headstart/504) if applicable

All guardians need to set up a my.DSD account with the district.

Kindergarten Registration

myDSD Guardian Account & Setup

In order to check your student out from school, access your student's schedule, pay fees, and so much more; all parents/guardians must have a myDSD account. 

How to get there:
Go to the top of this page near the upper right-hand corner click myDSD.

To create an account:
Click the Create an Account Button
Enter the email address ON AT THE SCHOOL
(If you don't know what the email address on file is, there is an option to enter your student's ID and PIN number.)
