Registration
Pick up a registration packet in the office
Complete the Student Registration Form.
Immunizations – Please provide the office with an original immunization record. If your student has not completed the required immunizations, they must be completed before starting school.
Birth Certificate – Must be a FULL SIZED ORIGINAL. Can NOT be wallet size birth card. You can obtain a copy from your current school in District or the Health Department.
Proof of Residency – We need two proof of address to show you are in our school boundaries. One needs to dated within the last sixty days. For ASC and DEEP Magnet, only the top portion will need to be filled out. A variance will be granted to those accepted into the programs and proof of residency will be waived.
Homeless Assistance Questionnaire (McKinney–Vento Eligibility) – Answer the two questions on the form.
Parent/Guardian ID
Guardianship Form – Fill out and sign.
I.E.P – (Resource/Speech/Preschool/Headstart/504) if applicable
